How it works

Four ways the form gets filled in

Unique email link with code

Email

Each client gets a branded invitation with your photo and a one-click button to their own private form. No logins, no passwords. One client credit the first time you email them. Resends are free.

We mail it for you

Direct mail

ShareMyRx prints the letter and mails it. A handwritten-style worksheet, a private access code, and a QR code to the same online form. One mail credit per letter.

Manual Input

You fill it in

Fill the same form yourself, on a call or from your notes. One client credit the first time that form is used. Uploading the client is free.

Direct copy access

Unique link

Copy a unique link to send or resend the form to clients. One client credit the first time that form is used.

It’s as easy as this.

Illustration of the agent dashboard: copy a pre-filled private link, then email, mail, or fill the form in yourself.

Then it all lands in one place

Hook it up to your CRM

Anywhere that can receive a webhook can receive your clients' data. Grab your platform's webhook and place it within ShareMyRx.

  • Email
  • Direct mail
  • You fill it in
  • Unique link

One submission. One contact record.

Medication lists, pharmacy, physician, and Medicare details map to the contact you already keep in GoHighLevel, LeadConnector, Zapier, or Make. CSV export is included. There is no plan to upgrade.

Features

Everything the old paper worksheet did, done right

ShareMyRx rebuilds the classic agent medication worksheet as a modern, reusable system, with the accuracy your plan comparisons depend on.

Medication search that understands drugs

Powered by the National Library of Medicine’s RxNorm with openFDA as backup. Clients pick the brand or generic, then the exact strength and dosage form.

Physician and pharmacy capture

Live NPI Registry lookup for the primary physician, plus the preferred pharmacy’s name, city, and state, so plan comparisons start with the right network.

Medicare details, validated

MBI format checking, Part A and Part B effective dates, and current coverage type (MA, PDP, or other) with carrier and monthly cost.

Designed for seniors

Large type with a text-size control, one question per row, clear required markers, high-contrast focus states, and a mobile-first layout.

Fits your CRM

Paste a webhook URL. Email, a letter we mail, a form you fill in, and website submissions all send the same payload to GoHighLevel, LeadConnector, Zapier, or Make the moment the form is complete.

Scope of Appointment, done for the year

One form, every channel, every client, each year. It lasts 12 months. You do not chase a separate Scope of Appointment later.

CSV in, CSV out

Import your book with flexible column mapping. Export submissions with one medication per row for plan comparison tools. CSV export is included.

Trust built in

Your face, your name, your agency

Seniors ignore anonymous forms. ShareMyRx puts your profile photo, title, and a short personal note at the top of every email, mailed letter, phone form, and generic form, so your clients know exactly who is asking and why.

  • Profile completeness checklist nudges you to add a photo and bio
  • Preview the exact email or letter before you send it
  • Client links are private, one per person, and expire 120 days after the last invitation

FAQ

Questions agents ask

How can clients fill the form?

Email a unique secure link, let ShareMyRx print and mail a letter, send your generic form link, or fill the same form in yourself on a call or from your notes. You can also copy a private link and send it yourself. Every path produces the same submission.

Do my clients need an account?

No. An emailed link asks for the access code in the email and their date of birth. A mailed letter uses that same code. A generic form link starts blank. There is no password.

Where does the medication data come from?

Drug names, strengths, and dosage forms come from RxNorm, maintained by the National Library of Medicine, with openFDA as a fallback. Physician records come from the CMS NPI Registry.

Is this compliant for Medicare marketing?

ShareMyRx is a data collection tool for your existing clients and does not market plans. Every agent signs a Business Associate Agreement with us, and you remain responsible for HIPAA safeguards and CMS marketing guidelines in your own communications.

Can I send the data to my CRM?

Yes. Paste a webhook URL from GoHighLevel, LeadConnector, Zapier, or Make in Settings. Email, a letter we mail, a form you fill in yourself, a copied link, and a website submission all post the same flat fields the moment they come in. CSV export is included too.

Does the form include a Scope of Appointment?

Yes. The same email, letter, website form, and manual visit collects a Scope of Appointment for the agent or agency on the profile. It is valid for 12 months from the day the client signs, so the paperwork is done for the year.

What do credits pay for?

A client credit is spent the first time that person’s form is released or submitted. A mail credit is spent only when a letter is mailed. Uploading a client is free.

Is there a subscription?

No. A new account starts with 10 client credits. After that you buy client credits and mail credits when you need them. There is no annual plan and nothing to cancel. Unused credits stay in your account.

Ready for a calmer AEP?

Set up your profile, upload your clients, and start collecting by email, a letter we mail, your website, or a form you fill in.
